You might want to use this:

We never applied for an actual street address that I know of.

I use the address Steve posted and it has worked fine for many years.
I also put down 1/1/2011 to 12/31/2011 for the dates.
For reason I put down, "monthly subgun matches"

As usual all the advice you gents have given me is correct and has saved me a lot of work and, I suspect, grief. My approved 5320.20s showed up in the mail today. Thanks.
